Psychological Assessment Services
Gatekeeping services focus on assisting individuals transitioning into or out of institutional settings, including state hospitals. The gatekeeper assigned to work with these individuals will assist with linkage to resources and ensure a safe transition back into the community. Mental health providers and/or state operated facilities are responsible for connecting individual with gatekeeping services when relevant. Gatekeeping services are available in all counties currently served by Valley Oaks Health.

Employment Services
Valley Enterprises is a an employment services program, funded by Vocational Rehabilitation, which specializes in assisting individuals with a mental health diagnosis in obtaining and maintaining employment in the local community. This service is available to residents of Tippecanoe County.

Transitional Residential Care
The transitional group home is a Lafayette area residence for individuals who are in need of a supportive living environment as a move toward greater independence. Residents are taught skills such as cooking, budgeting, and the use of community resources.

Bridges to Community Living
The Bridges to Community Living program consists of the Hybrid Program, Cluster Program, and Clubhouse and is available in Tippecanoe County.

Entitlement Services
The Entitlement Services Program assists VOH clients in pursuing state and federal entitlements. State and Federal entitlement categories include: Medicaid, HIP, Food Stamps, Social Security Disability and Supplemental Security Income. Staff help clients complete the application, file appeals and may attend appeal hearings when needed.

Batterers Intervention Program
Valley Oaks’ Batterers’ Intervention Program (BIP) focuses on victim safety and establishes accountability for batterers. During the program, offenders move from defining their behavior to understanding its implications and ending it.

Compass
The Compass Program is a new case management program for individuals who have previously been homeless, but now have housing. The goal of the program is to connect individuals to needed services and maintain housing. These services are available to residents of Tippecanoe County.

PATH
This is a grant program that serves people who are experiencing homelessness in Tippecanoe County. The goal of the program is to make connections with the individual, assess their needs and make needed referrals to address mental health, physical issues, housing and entitlements. Path case managers work on eliminating barriers and connecting individuals to Lafayette Transitional Housing to address their housing needs.

Child Forensics
The Child Advocacy Center is a resource provided in coordination with law enforcement and Department of Child Services (DCS) in Jasper and Newton Counties through our Rensselaer location. Other counties can be served upon DCS request. The CAC conducts forensic interviews with children between the ages of three to seventeen who have reported instances of sexual abuse/assault, physical abuse, and in some instances have witnessed a crime. The children and their families are referred to mental health services if necessary.

Beyond the Violence
Beyond The Violence is a grant funded free service available to all victims of domestic violence and sexual assault and their families. Beyond the Violence serves the Fountain and Warren County areas through our Attica location.

Severe Adult Needs
The ACT model is indicated for adults with severe and persistent mental illnesses that produce distress and major functional disability in adult role functioning (self-care, social and interpersonal relationships, employment, and navigating the community). This model includes a combination of intensive therapy, medication management, and case management services utilizing a multidisciplinary team approach. These individuals …
