Maham Mahmood, MD
Associate Medical Director
About Maham Mahmood
Dr. Maham Mahmood is a board-certified physician anesthesiologist in the Lafayette and Crawfordsville communities and has joined Valley Oaks Health as the Associate Medical Director of the Primary Care Division. Dr. Mahmood graduated residency from Indiana University School of Medicine in Indianapolis and now lives with her husband and daughter in Lafayette.
She is enthusiastic about helping people develop healthy habits and teaching others to provide high quality medical care. She currently holds the position of Adjunct Clinical Assistant Professor of Anesthesia with IU and Student Anesthesia Assistant liaison for Franciscan Health.
She is passionate about promoting holistic and preventative care in local communities and looks forward to serving all of the patients at Valley Oaks Health.

Family Nurse Practitioner
About Israel Nieto
Israel Nieto is a Family Nurse Practitioner with more than seven years of experience in bringing clinical expertise and a compassionate approach to care.
Before becoming a nurse, Israel served in the United States Marine Corps, where he developed the discipline and commitment that guide his work today.
He earned his Bachelor of Science in Nursing from Indiana University Kokomo and his Masters of Science in Nursing from Indiana Wesleyan University. Israel’s journey reflects a lifelong dedication to service and helping others.
We’re committed to serving all by providing quality care regardless of a patient’s financial standing. No one should be denied medical care because of their financial situation. Medicaid and Commercial Insurance are accepted. We also offer a sliding fee scale to further aid your financial needs.
